    Riluzole for relapse prevention following intravenous ketamine in treatment-resistant depression:a pilot randomized, placebo-controlled continuation trial

    The N-methyl-D-aspartate (NMDA) glutamate receptor antagonist ketamine may have rapid, albeit transient, antidepressant properties. This study in patients with treatment-resistant major depression (TRD) aimed to (1) replicate the acute efficacy of single-close intravenous (i.v.) ketamine; (2) test the efficacy of the glutamate-modulating agent riluzole in preventing post-ketamine relapse; and (3) examine whether pretreatment with lamotrigine would attenuate ketamine's psychotomimetic effects and enhance its antidepressant activity. Twenty-six medication-free patients received open-label i.v. ketamine (0.5 mg/kg over 40 min). Two hours prior to infusion, patients were randomized to lamotrigine (300 mg) or placebo. Seventeen patients (65%,) met response criterion (>= 50% reduction from baseline on the Montgomery-Asberg Depression Rating Scale) 24 h following ketamine. Lamotrigine failed to attenuate the mild, transient side-effects associated with ketamine and did not enhance its antidepressant effects. Fourteen patients (54%) met response criterion 72 h following ketamine and proceeded to participate in a 32-d, randomized, double-blind, placebo-controlled, flexible-dose continuation trial of riluzole (100-200 mg/d). The main outcome measure was time-to-relapse. An interim analysis found no significant differences in time-to-relapse between riluzole and placebo groups [log-rank chi(2) = 0.17, d.f. = 1, p = 0.68], with 80% of patients relapsing on riluzole vs. 50% on placebo. The trial was thus stopped for futility. This pilot study showed that a sub-anaesthetic close of i.v. ketamine is well-tolerated in TRD, and may have rapid and sustained antidepressant properties. Riluzole did not prevent relapse in the first month following ketamine. Further investigation of relapse prevention strategies post-ketamine is necessary

    Evaluation of plasma 3-methoxy-4-hydroxyphenylglycol

    A high-performance liquid chromatographic (HPLC) assay suitable for the evaluation of 3-methoxy-4-hydroxyphenylglycol (MHPG) in 1-ml aliquots of plasma is presented. Preparatory steps include extraction into ethyl acetate and minicolumn chromatography. Recoveries are monitored with [3H]MHPG. The HPLC procedure utilizes a C18 column, isocratic elution and amperometric detection. The assay was checked against a gas chromatographic--mass spectrometric procedure; the two procedures correlated well with a correlation coefficient of 0.99. Intra-assay reproducibility was 5.4%, inter-assay reproducibility 10.7%. Immediate changes in the orthostatic position did not affect the plasma MHPG concentration.     Saturn Atmospheric Structure and Dynamics

    2 Saturn inhabits a dynamical regime of rapidly rotating, internally heated atmospheres similar to Jupiter. Zonal winds have remained fairly steady since the time of Voyager except in the equatorial zone and slightly stronger winds occur at deeper levels. Eddies supply energy to the jets at a rate somewhat less than on Jupiter and mix potential vorticity near westward jets. Convective clouds exist preferentially in cyclonic shear regions as on Jupiter but also near jets, including major outbreaks near 35°S associated with Saturn electrostatic discharges, and in sporadic giant equatorial storms perhaps generated from frequent events at depth. The implied meridional circulation at and below the visible cloud tops consists of upwelling (downwelling) at cyclonic (anti-cyclonic) shear latitudes. Thermal winds decay upward above the clouds, implying a reversal of the circulation there. Warm-core vortices with associated cyclonic circulations exist at both poles, including surrounding thick high clouds at the south pole. Disequilibrium gas concentrations in the tropical upper troposphere imply rising motion there. The radiative-convective boundary and tropopause occur at higher pressure in the southern (summer) hemisphere due to greater penetration of solar heating there. A temperature “knee ” of warm air below the tropopause, perhaps due to haze heating, is stronger in the summer hemisphere as well. Saturn’s south polar stratosphere is warmer than predicted by radiative models and enhanced in ethane, suggesting subsidence-driven adiabatic warming there.     Au fur et à mesure que la conscience collective s’est éveillée à l’égard des enjeux environnementaux jusque dans les gestes les plus quotidiens, l’idée s’est fait jour que l’écologie est appelée à se présenter comme un projet politique à part entière. Pour envisager un tel projet, il convient toutefois de se donner des moyens de pensée qui l’élèvent à la hauteur d’autres grands modèles politiques d’organisation sociale ; des moyens qui impliquent en outre la transition, le passage d’un modèle à l’autre. La réflexion nous a conduit à aller chercher ces moyens là où l’on ne se souciait plus guère, ces temps-ci, de les trouver : dans des théories linguistiques. Dans cette conférence, on montrera l’efficacité de concepts linguistiques pour la pensée du politique. Une lecture de La Démocratie en Amérique II (1840) reconstruira sous la forme de systèmes spécifiques de rapports sociaux — des systèmes « participatifs », ainsi que les a conçus le linguiste Hjelmslev — la manière dont Tocqueville distingue les sociétés démocratiques des sociétés aristocratiques, puis nous conduira à élaborer, sur ce canevas, un système écologique. On observera en outre comment les modalités d’action (vouloir, pouvoir, devoir), comme la linguistique les analyse, sous-tendent la logique de ces systèmes en offrant le moyen de distinguer différentes stratégies d’action pour l’organisation sociale
